Together, you’ll review the doctor’s recommendations and schedule your ongoing visit frequency to best support your goals and care plan.We will craft a care plan individual to your needs and give you a road map to get better and stay better.

before you arrive

Before you arrive, plan to show up about 10 minutes early so we can have you fill out new patient paperwork. There’s nothing you need to bring— just wear comfortable clothes and come as you are.

Any cancellations made less than 24 business hours before the visit will be subject to the full cost of the current fee schedule pricing.  Any visits not cancelled or rescheduled within the 24 business hours are considered a no-show and will be subject to the entire visit cost.
